ABI Group is seeking a compassionate and bilingual Social Worker to support patients and caregivers with essential benefits such as Medicaid renewals, SNAP/food stamps, housing assistance, and other social services. This role ensures patients remain enrolled in the programs they depend on, while strengthening long-term trust and retention.
The ideal candidate will be fluent in English and Mandarin, or Cantonese (other dialects preferred), and have experience helping families navigate benefits and community resources
Key Responsibilities
● Serve as the primary case manager for patients and caregivers seeking benefits support.
● Assist with Medicaid renewals, SNAP, housing, and other applications.
● Maintain accurate case files and records to simplify recurring renewals.
● Provide proactive outreach and reminders to ensure deadlines are met.
● Connect patients and caregivers with community resources and advocate on their behalf.
● Collaborate with ABI Group staff to identify high-need patients and prioritize support.
● Track service utilization and outcomes to guide program improvements.
Qualifications
● Bachelor’s degree in any field (Master’s degree in any field preferred).
● Licensed Social Worker (LMSW/LCSW) in New York State required; strong case manager experience will also be considered.
● Fluent in English and Mandarin, or Cantonese
● Prior experience with Medicaid, SNAP, housing, or benefits navigation.
● Excellent communication and organizational skills; ability to manage multiple cases.
● Compassionate, patient-centered, and culturally sensitive.
● Must be able to work across multiple locations within Queens
● Field visits may be required, including home visits, and patient follow-up visits as needed.
Compensation & Benefits
● Competitive salary (commensurate with experience): $67,000 - $80,000
