Schedule: Monday-Friday 8:30 a.m. - 5:00 p.m.
Job Summary: The Housing Solutions Case Manager, under the direction of the Housing Solutions Manager, is responsible for assisting Mercy House clients in obtaining and sustaining permanent housing by utilizing the Housing First Model. This position is responsible for providing case management services and housing advocacy to clients that enter our system of care. The Housing Solutions Case Manager role assesses clients, evaluates effectiveness of housing plans, and tracks progress made by clients towards the clients’ housing goals. This position advocates for and supports the clients enrolled in the Rapid Re-housing Program for homeless individuals.

Essential Duties and Responsibilities:
Program Oversight
- Attend program meetings as assigned
-
Provide comprehensive case management: intakes, housing stabilization plans, goal setting, service linking, and progress monitoring

Use client-focused strategies and harm reduction methods to support housing stability
-
Apply Trauma Informed Care and Motivational Interviewing
-
Maintain case notes using the GIRP (Goal, Intervention, Response, Plan) format
-
Offer tenant education, advocacy, and referrals
-
Develop individualized case management plans, including financial and housing goals
-
Meet regularly with clients to monitor progress and ensure compliance with program requirements
-
Empower clients in a supportive, objective, and client-centered manner
-
Keep case files updated and enter data into HMIS promptly
-
Conduct home visits and habitability inspections as needed
-
Assist clients with employment and housing barriers
-
Help clients identify strengths and address needs respectfully
-
Maintain knowledge of and refer clients to community resources

Follow all agency case management policies
-
Manage caseload as assigned

Understand and follow Coordinated Entry procedures; connect clients to shelter if needed

Support clients in housing searches, provide leads, and facilitate landlord relationships
-
Mediate landlord/resident issues to support housing retention

Complete re-certifications and collect documentation on time

Follow up with clients as directed
Team Tasks

Collaborate with staff, interns, and volunteers to support agency mission and values

Attend staff meetings, supervision, community meetings, and trainings

Participate in monthly Housing Solutions team meetings to discuss client progress and share resources

Work with leasing agents and support client housing efforts

Administrative

Maintain accurate and current client documentation (plans, case notes, call logs, emergency plans, etc.)

Submit backup documentation through Neighborly software promptly

Support program activities and data collection

Build partnerships with other agencies

Protect client confidentiality and understand HIPAA regulations

Ensure timely and valid submission of client documents

Use computers and software effectively

Respond promptly to calls and emails

Work independently and prioritize tasks and deadlines

Occasionally lift items up to 25 lbs and sit for extended periods

Complete other tasks as assigned by Housing Solutions Supervisor or Manager
Qualifications: Bachelor’s degree in social work or related field and/or four years of experience in social work or human services in a public or private organization is a requirement. Experience working with the homeless population is highly preferred. The candidate must be proficient in all Microsoft Office programs and must possess basic typing, email, and internet skills. A valid driver’s license, proof of car insurance and a reliable vehicle is required. Bilingual in Arabic, English, Farsi, Spanish, or Vietnamese is highly preferred.
