Job Summary – Hybrid MDS Coordinator (RN)

The MDS Coordinator (Registered Nurse) is responsible for coordinating and completing the Minimum Data Set (MDS) process in compliance with CMS regulations and state guidelines. This role ensures accurate clinical documentation, comprehensive care planning, and regulatory compliance within the Skilled Nursing Facility.

Key Responsibilities

  • Oversee and complete the full MDS process in accordance with CMS and state regulations.

  • Develop, coordinate, and update comprehensive resident care plans.

  • Ensure accurate MDS coding and supporting clinical documentation.

  • Conduct routine chart audits and documentation reviews.

  • Collaborate with the interdisciplinary team to support care plan implementation.

  • Monitor resident condition changes and initiate appropriate assessments.

  • Assist with discharge planning and required regulatory reporting.

  • Maintain compliance with infection control policies and safety standards.

  • Protect resident health information and maintain HIPAA compliance.

  • Participate in care conferences, QA meetings, and interdisciplinary meetings.

Qualifications

  • Active North Carolina RN License (Registered Nurse)

  • CPR Certification

  • Knowledge of CMS regulations and the MDS 3.0 process

  • Experience with care planning in a Long-Term Care or Skilled Nursing Facility

  • Strong clinical documentation and auditing skills

  • Excellent communication and organizational abilities

  • Minimum one year of RN experience in Long Term Care or Skilled Nursing

  • Prior experience as an MDS Coordinator, RNAC, or Clinical Reimbursement Nurse preferred
