Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health status by observing overall condition, reporting any concerns promptly
• Prepare nutritious meals tailored to individual dietary needs and preferences, ensuring safe food handling practices
• Support clients with cognitive impairments such as dementia or Alzheimer's disease through engaging activities and memory care techniques
• Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing personal space
• Document care activities meticulously in compliance with healthcare regulations and care plans
• Collaborate with healthcare professionals to implement personalized care strategies and adjustments as needed
• Provide emotional support and companionship, fostering a positive environment that promotes mental health and social engagement

Requirements

• Proven experience in home & community care, assisted living, nursing homes, or group homes is highly desirable
• Valid certifications in First Aid and CPR; additional training in hospice or dementia care is a plus
• Knowledge of HIPAA regulations to ensure client confidentiality at all times
• Ability to work independently while demonstrating excellent communication skills with clients and team members
• Physical stamina for tasks involving heavy lifting, patient transfers, or long periods of standing and walking
• Experience working with individuals with developmental disabilities, autism, or memory impairments preferred
• Strong observational skills for patient monitoring and identifying changes in health status
• Flexibility to adapt to different client needs within various home environments
• Compassionate attitude combined with a proactive approach to delivering high-quality caregiving services
