Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and record patients’ vital signs, health status, and behavioral changes to ensure optimal care
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support medication administration following care plans and HIPAA privacy standards
- Conduct patient observation to identify any health concerns or emergencies promptly
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment
- Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility assistance
- Collaborate with healthcare professionals to develop or update personalized care plans for each client
- Provide emotional support and companionship to enhance mental well-being and reduce feelings of isolation
- Follow all safety protocols including CPR and First Aid procedures to respond effectively during emergencies
Experience
- Prior experience in home & community care or assisted living environments is highly preferred
- Familiarity with working with individuals with developmental disabilities, Alzheimer’s disease, dementia, autism spectrum disorder, or hospice care needs
- Knowledge of patient monitoring techniques, medication administration, and vital signs assessment
- Experience with long-term care settings such as nursing homes or group homes is advantageous
- Demonstrated ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Strong understanding of HIPAA regulations to maintain client confidentiality at all times
- Proven skills in providing direct support and compassionate caregiving tailored to individual needs
- Valid certifications in CPR and First Aid are required; additional certifications related to dementia or memory care are a plus
