Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, noting changes in condition or behavior for care plans
- Prepare nutritious meals tailored to dietary needs and assist with meal consumption when necessary
- Administer medications accurately following prescribed schedules and document medication administration in compliance with HIPAA regulations
- Support patients with mobility using equipment such as Hoyer lifts or other assistive devices; perform heavy lifting when required
- Maintain a clean and safe environment through regular cleaning, laundry, and organization of living spaces
- Follow established care plans and communicate effectively with healthcare professionals, family members, and team members
Skills
- Proven experience in home health, assisted living, nursing home environments, or group homes with a focus on senior or disability care
- Knowledge of patient observation techniques, vital signs monitoring, and dementia or Alzheimer’s care strategies
- Certifications such as CPR, First Aid, HIPAA compliance training preferred; ongoing training encouraged
- Experience working with individuals with developmental disabilities or autism spectrum disorder (ASD)
- Ability to perform light to heavy lifting safely using proper techniques; familiarity with Hoyer lifts is a plus
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Compassionate attitude combined with patience and adaptability to meet diverse client needs
