Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by checking vital signs and observing changes in condition; report findings accurately and promptly
• Administer medications following care plans and HIPAA regulations to ensure safety and compliance
• Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation
• Utilize specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility support
• Maintain a clean, safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces
• Support clients' emotional well-being through companionship, engaging activities, and social interaction

Skills

• Proven experience in home & community care or assisted living environments with a focus on senior care or long-term care
• Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or autism
• Certified in First Aid and CPR; familiar with medication administration and vital signs monitoring
• Ability to perform heavy lifting safely when required; experience with resident’s <PERSON> (personalized care plans) is a plus
• Strong understanding of HIPAA regulations to ensure client confidentiality at all times
• Excellent communication skills for effective collaboration with families, healthcare professionals, and team members
• Compassionate attitude combined with the ability to handle challenging situations calmly and professionally
