What You'll Do

• Conduct physical therapy evaluations and reassessments
• Create treatment plans tailored to patient goals
• Improve patient mobility and safety
• Educate patients and families on exercise and fall prevention
• Coordinate care with the interdisciplinary team
• Maintain Medicare-compliant documentation

What We're Looking For

• Hawaii Physical Therapist license
• One year of clinical experience
• Excellent communication skills
• Ability to work independently
• Reliable transportation
