In-Home Visits Nurse Practitioner

Job Overview
We are seeking a dedicated and energetic In-Home Visits Nurse Practitioner to provide comprehensive healthcare services directly in patients' homes. Duties

  • Conduct thorough patient assessments during in-home visits, including vital signs, physical examinations, and medical history reviews.
  • Develop and implement individualized care plans tailored to each patient's needs, considering chronic conditions and acute issues.
  • Administer treatments such as injections, IV infusions, botulinum toxin therapy, dermal fillers, and other aesthetic laser procedures when appropriate.
  • Manage medication administration, including injections and oral therapies, ensuring proper documentation in electronic health records (EHR) systems like Epic or eClinicalWorks.
  • Perform diagnostic evaluations utilizing sonography, laboratory specimen collection/processing, and other medical imaging techniques as needed.
  • Provide health coaching and education to patients and families on disease management, medication adherence, nutrition, and lifestyle modifications.
  • Collaborate with primary care physicians, specialists (e.g., gastroenterology, infectious disease), and case managers to coordinate comprehensive patient care plans.
  • Maintain compliance with HIPAA regulations and ensure accurate medical documentation for all patient interactions.
  • Assist with triage over the phone or via telehealth platforms to evaluate urgent needs and determine appropriate next steps.
  • Support discharge planning, post-acute care transitions, and utilization review processes to optimize patient outcomes.

Requirements

  • Current Nurse Practitioner license with active certification to practice independently in the state of employment.
  • Proven experience in acute care settings such as ICU or emergency medicine; hospital or nursing home experience highly preferred.
  • Strong clinical skills including physical examinations, suturing, catheterization, phlebotomy, sterile processing techniques, and airway management.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for efficient documentation.
  • Knowledge of CPT coding, ICD-10/ICD-9 coding standards for billing and medical records management.
  • Experience working with diverse populations including pediatrics (pediatrics), geriatrics (memory/dementia care), assisted living residents or individuals with developmental disabilities.
  • Ability to perform complex procedures such as spinal taps or cardiac catheterizations when necessary; experience in telehealth is a plus.
  • Excellent communication skills for patient education and interdisciplinary collaboration; critical thinking for diagnostic evaluation and case management.

Pay: $180-200 per diem visit
