Position Summary:

As a Wound Care focused Nurse Practitioner, you will practice with true autonomy, manage complex clinical cases, and build long-term relationships with patients you see regularly. Responsibilities:
  • Provide on-site rounding, primary care, and advanced wound care services.
  • Manage complex wounds including pressure injuries, ulcers, and post-acute cases.
  • Drive preventative care, chronic disease management, and care coordination.
  • Partner with physicians, nurses, and facility teams to deliver high-level care.
  • Reduce hospitalizations through proactive, hands-on clinical management.
  • Communicate with families and caregivers to build trust and alignment.
  • Oversee documentation, medication management, and lab coordination.


Qualifications:
  • Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
  • Experience in geriatrics, wound care, primary care, or post-acute care preferred.
  • Valid driver’s license and ability to travel between communities.
  • Strong desire for independence, growth, and expanded clinical responsibility.
  • Passion for making a measurable impact in senior care.

  • Army: Nurse Practitioner (66P).
  • Navy: Nurse Practitioner (1972).
  • Air Force: Nurse Practitioner (46Y3H).
  • Marines: Nurse Practitioner (NP).
  • Coast Guard: Health Services Technician (HS).
