Staff Pharmacist

Job Description

Role Purpose

The Staff Pharmacist collaborates with the Pharmacist in Charge to ensure prescriptions

are accurately and efficiently processed while delivering high-quality patient care. This role

also supervises pharmacy staff, ensures compliance with policies and regulatory

requirements, and may serve as Pharmacist in Charge in the absence of the PIC.

Qualifications / Requirements

  • Active pharmacist license in the state of practice (required); Home infusion

experience preferred.

  • Knowledge of sterile compounding, cleanroom operations, and regulatory

compliance.

  • Strong clinical judgment, organizational, and communication skills.
  • Proficiency with pharmacy management systems, documentation, and reporting.
  • Active licensure and up-to-date completion of all required training, competencies,

and certificates.

  • Participation in rotating on-call coverage as defined by the On Call Policy.

Primary Responsibilities / Accountabilities

The Staff Pharmacist is responsible for completing all pharmacy tasks accurately,

efficiently, and in compliance with regulatory, facility, and internal protocols.

Responsibilities include, but are not limited to, the following categories:

1. Patient Care

  • Review new referrals for completeness and accuracy.
  • Review medication profiles and run interaction checks when appropriate.
  • Review allergies and pertinent medical conditions.
  • Create DWOs and ensure all patient profiles are complete.
  • Ensure all required information is obtained prior to servicing a patient (IV access,

signed orders, following physician, and nursing plan).

  • Complete counseling, home safety evaluations, and initial assessments for every

patient.

  • Provide patient counseling on medications as appropriate, including when therapy

is initiated, modified, or upon patient request.

  • Review and complete follow-up assessments, taking appropriate actions.
  • Review and update care plans regularly.
  • Track, review, and document laboratory results to support appropriate therapy

monitoring.

  • Review nursing notes and take appropriate actions.
  • Document all patient files accurately (progress notes).
  • Discharge patients when appropriate.

2. Workflow & Prescription Processing

  • Manage CareTend queues, daily tasks, and workflow to ensure all activities are

completed accurately and on time.

  • Complete all steps of production verification.
  • Verify drug, diluent, container selection, and compounding calculations for

accuracy prior to entering the cleanroom.

  • Create all initial prescriptions and print or schedule accordingly.
  • Prepare and print all TPN prescriptions (initial and refills).
  • Program infusion pumps (final check must be a pharmacist).
  • Complete verification of anaphylactic kits and supply kits.
  • Supervise cleanroom activities and sterile technique.
  • Perform sterile compounding as needed.
  • Ensure compliance with all pharmacy policies, procedures, and competencies

during workflow.

  • Ensure completion of all applicable On-Track tasks on time.
  • Participate in any pharmacy activity as necessary to support operations.

3. Documentation

  • Record occurrences for Outcomes Processing.
  • Ensure all admission paperwork is received prior to servicing a patient.
  • Ensure DWOs are signed and returned timely.
  • Assist with resolving grievances as requested by management.
  • Assist with administrative reports as requested by management (e.g., Risk Reports,

Root Cause Analyses, HIPAA Disclosure Forms).

4. Communication

  • Communicate with doctors’ offices for verbal orders, order clarification, and therapy

updates.

  • Recommend therapies and offer solutions to clinics and patients when appropriate.
  • Act as a resource for clinics, nurses, and patients.
  • Compose clinical prior authorization letters as appropriate.

5. Supervision

  • Supervise activities of all pharmacy personnel.
  • Advocate for timely admission of new patients.
  • Act as a leader in the pharmacy and support the PIC by ensuring:

o Accurate, timely, and efficient workflow.

o Proper documentation of all files.

o High-quality patient care and customer service.

o Compliance with all policies and procedures.

  • Serve as point of contact for corporate leadership, regulators, and auditors

regarding pharmacy operations in the absence of the PIC.

Authority & Escalation

Authority:

  • Perform all assigned pharmacy tasks independently and accurately.
  • Make operational and clinical decisions regarding assigned tasks within standard

protocols.

  • Supervise and direct pharmacy staff during assigned shifts to ensure safe and

compliant pharmacy operations.

  • Follow all policies, procedures, and workflow requirements established by the PIC.

Escalation:

  • Immediately escalate deviations from established protocols, regulatory

requirements, or unsafe practices to the PIC or Lead Pharmacist.

  • Report noncompliant or potentially harmful practices affecting patient safety, staff

performance, or pharmacy operations.

  • Seek guidance when unsure about task requirements, unusual situations, or

potential conflicts in workflow or patient care.

Daily / Ongoing Responsibilities

  • Maintain oversight of daily pharmacy operations, ensuring workflow efficiency and

compliance.

  • Monitor staff performance and provide coaching and guidance as needed.
  • Ensure proper documentation, recordkeeping, and reporting of pharmacy activities.
  • Maintain facility readiness, cleanliness, and safety standards.
  • Monitor and ensure compliance with regulatory requirements and internal policies.
  • Complete all required training, competencies, and certifications on time.
  • Fulfill assigned on-call and after-hours duties as required.

Key Performance Indicators / Success Metrics

  • Prescriptions and pharmacy tasks are completed accurately and on time.
  • Patient care tasks, documentation, and follow-ups are complete and compliant

with internal and regulatory standards.

  • Sterile compounding, cleanroom, and environmental protocols are consistently

followed.

  • Inventory, records, and reports are accurate and up to date.
  • Pharmacy staff are competent, confident, and compliant with all protocols.
  • Patient care and satisfaction meet or exceed expectations.
  • On-call and after-hours tasks are executed reliably and in compliance with

company policy.

Key Interactions / Relationships

  • Pharmacist in Charge – daily operational oversight, guidance, and escalation.
  • Pharmacy Technicians – supervision, task delegation, training, and workflow

support.

  • Other clinical staff – coordination on medication delivery and patient care.
  • Vendors / Service Providers – coordination for inventory, recalls, and maintenance

tasks

Job Type: Full-time

Pay: $65.00 - $68.00 per hour
