Position Summary: Provide triage for patient/family phone calls and coordinate any necessary follow up with the Interdisciplinary Group (IDG) team. Initiates home care referrals over the phone from physician offices, hospitals, SNF, community agencies and community at large. Serves as a customer resource and arranges appointments for evaluation as necessary. Provides comprehensive support for field clinicians, patients, families, providers, managers and internal staff.
- A graduate of an approved school of professional nursing, who is a Licensed New York State Registered Nurse (RN)
- Experience in interviewing, assessment, communication, negotiating and decision-making skills.
- Minimum of one year of experience in Home Care in a CHHA or related experience. Hospice experience preferred.

Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodation:
Triage:
- Triages calls from hospice patients and families/caregivers to gather information related to patient/family needs and/or symptom management.
- Provides nursing education to patients/caregivers based on patients’ plan of care and IDG recommendations.
- Addresses medication inquiries/refills/changes, DME and supply needs, or need for home visit by a hospice nurse.
Referral Management:
- Obtains and completes referrals over the phone for UR Medicine Hospice services. Identifies and documents the hospice focus of care, hospice diagnosis, prognosis, and essential demographic information.
- Ensure insurance payors have been verified.
- Communicates and coordinates referral process with the interdisciplinary team at UR Medicine Hospice and at institutions, as well as with patients/families. Involves patient and family support system in the referral process to assure the maximum involvement in patient care needs at home, if appropriate.
- Ensures documentation meets regulatory, institution and agency/departmental standards.
- Referrals are timely, accurate, and complete and communicated to the hospice team.
- Electronic Medical Record (EMR) activities to include referral entry, name changes, demographic changes, NTUCs and processing deaths.
Scheduling:
- Coordinates field staff schedules utilizing EMR scheduling tool. Ensure patients are scheduled for home visits according to departmental standards and processes.
- Assist nurses with arranging unscheduled visits daily as needed.
- Schedule and/or assist nurses with scheduling evaluation and admission visits for hospice enrollments in all settings as needed.
- Provides weekend staff with scheduling needs to ensure timely admissions and/or patient visit needs.
Compliance:
- Comply with agency and regulatory standards.
- Completes required/mandatory in-services, professional licensing requirements, employer and institutional requirements.
- Answers phone calls within agency standard guidelines.
- Read and respond to emails throughout workday.
- Comply with Corporate Compliance standards including confidentiality and HIPPA requirements.
- Comply with safe work practices (ergonomics, OSHA).
- Shares in responsibility for team maintenance of logs, statistics, and report corrections.
- Take responsibility for professional and personal growth and development.
- Attend team meetings and in-services.
- Participates in mentoring/training new staff.
- Attends educational opportunities and shares knowledge with others.
- Works with team to assure maximal efficiency of resources.

Salary Range: $67,700 - $111,717
