Essential Functions:
Responsibilities: To review each assigned participant’s assessment and Individualized Program Plan, (IPP), and facilitate their securing and maintaining of employment. To document participant performance and provide feedback to funding sources and other interested parties with a right to know. To develop job placement sites in the community. This is a field-based position that requires reliable transportation to travel regularly to participant work sites and employer locations
.
Skills and Abilities:
Requirements:
1. High School diploma or equivalent required, degree in Social Service field preferred.
2. Experience working with individuals with disabilities.
3. Must pass criminal background check.
4. Must be able to clear DMV check.
5. Must provide own transportation.
6. Must possess valid driver’s license and maintain current vehicle registration, liability insurance and vehicle inspection.
Physical/Mental Demands:
Must be able to tolerate frequent periods of high stress, heavy workload, and multiple priorities. Must be ambulatory. Must be able to see, hear, speak, and otherwise communicate effectively with participants, employees, and staff. Must be able to stand, sit, walk, climb, balance, kneel, crouch, crawl, reach, handle and possess depth perception.
