Position Summary

The JTOP Case Manager is responsible for providing comprehensive case management services to justiceinvolved individuals participating in the Justice Targeted Outreach Program (JTOP). This role supports clients with care coordination, service linkage, courtrelated compliance, and individualized treatment planning while collaborating closely with internal clinical staff and external justice partners. The JTOP Case Manager plays a key role in supporting client stability, treatment engagement, and successful community reintegration.

Essential Duties and Responsibilities

  • Provide case management services to justiceinvolved clients enrolled in JTOP, including assessment, planning, coordination, monitoring, and advocacy.
  • Complete intake assessments and ongoing reassessments to identify client needs, barriers, strengths, and goals.
  • Develop, implement, and maintain individualized service plans in collaboration with the client and treatment team.
  • Coordinate care with internal clinical teams, peer support staff, and external providers.
  • Maintain communication with justice system partners, including courts, probation officers, parole officers, and diversion program representatives, as required.
  • Assist clients with linkage to community resources such as housing, employment, benefits, transportation, medical care, and behavioral health services.
  • Support client compliance with courtordered or programrequired conditions.
  • Provide crisis intervention and deescalation support as needed, following Sanctuary Recovery Centers policies and procedures.
  • Accurately document all client interactions, services provided, and case notes in the electronic health record in a timely and compliant manner.
  • Maintain confidentiality and compliance with HIPAA, 42 CFR Part 2, ADHS, and AHCCCS requirements.
  • Participate in case staffing, multidisciplinary team meetings, supervision, and required trainings.
  • Perform other related duties as assigned to support program operations.

Required Qualifications

  • Bachelor's degree in Social Work, Psychology, Behavioral Health, Criminal Justice, or a related field OR equivalent relevant experience as permitted by ADHS and AHCCCS guidelines.
  • Experience working with justiceinvolved populations, individuals with substance use disorders, mental health conditions, or cooccurring disorders preferred.
  • Knowledge of community resources, behavioral health systems, and justice system processes.
  • Ability to maintain professional boundaries and work effectively with diverse populations.
  • Strong organizational, documentation, and timemanagement skills.
  • Basic computer proficiency and ability to use electronic health record systems.
  • Valid Arizona driver's license and reliable transportation (if client transport or community work is required).
  • Must pass background checks, fingerprint clearance (as required), and preemployment screenings.

Preferred Qualifications

  • Prior experience in justicerelated programs (probation, parole, diversion, specialty courts).
  • Knowledge of ADHS and AHCCCS requirements for case management services.
  • Bilingual (Spanish/English) preferred but not required.

Physical RequirementsWork Environment

  • Ability to sit, stand, walk, and move throughout the facility and community locations.
  • Ability to lift up to 25 pounds as needed.
  • May involve communitybased work, court attendance, and travel between sites.
