Kidney Transplant Assistant

Job Description:

The Transplant Assistant II - Transplant Navigator facilitates effective clinic organization and patient care by serving as a communication link between the patients, staff, physicians, hospital departments or other outside health care entities. Ensures compliance to CMS requirements for communication to families and donors. Develops and maintains systems for data management to meet requirements and for reporting to internal and external consumers of the information. Is responsible for navigating patients through multiple testing, consultations and / or treatments that patients receive in transplant through ongoing and extensive interaction with the patients and transplant coordinators. Assists in the determination of the most appropriate strategies and logistics to execute the plan of care that is initiated and outlined by the transplant provider. Navigators are responsible for understanding and applying treatment plans, SOP's and disease specific criteria to each patient, in order to assist the patient in moving through the transplant process, or in acute management of organ failure.
The Transplant Assistant II - Transplant Navigator is assigned to a specific department supporting one or more patient populations and the transplant care management team. Duties may vary by setting and they generally work under limited supervision, while acting on their own initiative and direction. Typically, the Transplant Assistant may orient and educate other clerical positions. The position interacts with patients to gather information that is used by others for their treatment and maintains complete and timely medical records and reports. They correspond with patients, their families, insurance providers, etc. in a professional and effective manner and communicate effectively and work collaboratively with physician(s), utilizing the chain of command appropriately as needed. Assists with insurance prior authorization for procedures / transplants / medications with payers, receiving and prioritizing requests and sending clinical information for authorization of services and provides triage for all incoming calls. Maintains an adequate inventory of supplies, office supplies, patient education materials and equipment to meet clinic requirements.
Transplant Assistant II - Transplant Navigator collaborates with clinicians to coordinate and track medical care,
ensuring patients’ complete health maintenance and transplant related testing are completed and reported.
Collaborates with team members to maintain appropriate clinic load, and appropriately identifies and accommodates special needs. Communicates with patients on plan of care and routine test findings and manages the logistics of a patient's on-going pre- and post-transplant care, including evaluations at certain milestones, varied procedures /
testing / results and discussions of progress. Collects pertinent information from family donors and / or volunteer
donors and the process of arranging for appropriate lab testing locally or nationally. Participates in marketing the
program, community awareness and education, and supportive activities to increase volume and enhance patient
satisfaction. Understands, educates, supports and acts as a liaison to referring physicians and contributes to program databases regarding protocol and patient data.

Shift Details: Full time, 40hrs/wk. Monday through Friday, generally between the hours of 7:30 am-5:00pm (business hours) with some minor flexibility in schedule. Ability to work in the office is required.

Minimum Qualifications

If working at Primary Children's Hospital, requirements include either a Certified Nurse Assistant (CNA) or Medical

Assistant (MA) and a Basic Life Support Certification (BLS) for healthcare providers.

If working at Intermountain Medical Center, requirements include a Basic Life Support Certification (BLS) for healthcare providers. (as needed)

Demonstrated experience working in a complex role with patient coordination and tracking responsibilities.

Demonstrated staff or customer service leadership experience utilizing computer skills including word processing,

spreadsheet applications, email, appointment scheduling, registration, billing, and electronic medical records.

Demonstrated effective interpersonal and communication skills.

Strong analytical, writing, and critical thinking skills.

Demonstrated ability to organize, coordinate, prioritize and work independently on projects.

Demonstrated general office skills (i.e. typing, faxing, and filing).

Demonstrated expertise in department specific competency’s or job skills.

Preferred Qualifications

College course related to healthcare, business, and / or computer/information systems.

Medical Assistant certification Level II or III, or five years CNA experience in a mixed hospital and clinic setting with working in a complex role patient coordination and tracking responsibilities.

Two years of experience in a Transplant Assistant or similar health care role including: scheduling, reception, medical records, filing, prior authorization and insurance authorization, Medical Terminology and telephone triage.

Leadership experience (i.e. providing training to others in their job duties, coordinator the work of others, coordinating projects, acting as a team lead Experience in a Transplant Assistant or similar health care role including: scheduling, reception, medical records, filing, prior authorization and insurance authorization, Medical Terminology and telephone triage.

Spanish speaking / interpreter skills.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$21.84 - $33.23
