Summary
Hours: 5:00 a.m. – 1:30 p.m. 8-hour, PRN
Under the supervision of a Wound Care RN, the Licensed Practical Nurse (LPN) – Treatment Team utilizes a general understanding of the principles of nursing and basic physical assessment skills in the development and implementation of individualized nursing care plans to ensure that the needs of the residents are met. The LPN assists in the orientation and supervision of nursing personnel, attends to the daily operations of the unit on a per-shift and unit level, and assumes responsibilities of a leadership role as needed.
Data Collection
In conjunction with the Wound RN, conducts ongoing reassessment and gathers data relevant to the individual needs of the patient or family.
Performs admission and ongoing focused system measurements, as appropriate to specialty; obtains patient history, including objective and subjective data.
Observes patient’s clinical condition; reviews diagnostic information; recognizes implications of abnormal diagnostic results; identifies problems typically associated with a diagnosis or treatment; reports pertinent information to appropriate personnel for further direction.
Performs individual patient care according to established plan of care and standards of practice.
Identifies learning needs and abilities of patient and family; implements teaching plan.
Identifies subtle and early indicators of crisis situations and reports to appropriate personnel.
Following the initial plan of care completed by the RN, reviews and updates care plans per policy.
Collaborates with other staff members to update the patient’s plan of care to ensure optimal, cost-effective, and high-quality patient care according to policy.
In collaboration with the RN, identifies and addresses discharge planning needs of the patient/family/significant other.
Implements, documents, and provides care and education based on the plan of care.
Responds to and actively participates in emergency codes.
Displays flexibility and teamwork by readily accepting patient assignments and unanticipated changes in daily routines, as well as additional duties to meet unit needs.
Provides safe, effective, and efficient wound and skin care to assigned group of patients.
Attends multidisciplinary rounds, acting as patient advocate and providing patient and family information, as appropriate.
Delivers care in a cost-effective manner through appropriate usage of patient care supplies.
Evaluates and documents patient care outcomes.
Monitors assigned patients. Based on observed data, identifies actual and potential problems and informs RN of findings.
Documents the patient/significant other’s response to the teaching plan.
Documents ongoing data, interventions, and observed patient responses. Assists RN in the evaluation of patient care.
Demonstrates professionalism, integrity, and accountability in the LPN role.
Accepts the shared governance philosophy and actively engages in peer review, self-scheduling, and other facility activities.
Education/Knowledge:
Formal working knowledge equivalent to an Associate’s degree (2 years of college); requires knowledge of a specialized field.
Associate’s degree preferred.
Experience:
1–3 years of experience preferred.
Certifications/Licenses:
Current Maryland License for Practical Nursing (LPN).
Current AHA BLS/CPR Certification.
