Licensed Practical Nurse (LPN)

The Licensed Practical Nurse (LPN) is responsible for delivering personalized, physician-directed healthcare services to individuals in a home or clinical setting. The role involves hands-on patient care such as administering medications, monitoring vital signs, managing wounds, and supporting daily activities. The LPN also educates caregivers, collaborates with other medical professionals, and ensures the comfort and well-being of patients, including those with specialized care needs.

Key Responsibilities:
Healthcare Plan Management
  • Implement patient care plans under physician direction
  • Coordinate with healthcare personnel to enhance patient outcomes
Personalized Care
  • Assist with bathing, grooming, dressing, and daily living activities
  • Provide palliative care to support comfort and quality of life
Vital Sign Monitoring
  • Track and record vital signs (temperature, pulse, blood pressure)
  • Watch for abnormalities and report findings
Medication Administration
  • Administer prescribed medications accurately
  • Maintain detailed and compliant medication records
Wound and Infection Care
  • Identify and treat signs of infection, pressure sores, and wounds
  • Deliver appropriate interventions and report to healthcare team
Caregiver Education
  • Train family or in-home caregivers on safe patient handling and daily routines
  • Offer guidance to reinforce effective care techniques
Specialized Clinical Skills (Case-Dependent)
  • Peg Tube/G-Tube care experience
  • Trach/Vent care proficiency
Qualifications:
Education:
Completion of accredited LPN program

Experience:
Minimum one year of nursing experience

Required:
Valid In-State or Multi-State RN license·

CPR/BLS certification

Current TB test

Reports to:
RN Case Manager
