Licensed Practical Nurse

Job Summary

Georgia Essential Private Care is seeking a compassionate, clinically skilled, and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care to pediatric patients (birth through age 21) in the comfort of their homes.

Our patients are medically fragile children and adolescents who may be wheelchair-bound and/or dependent on tracheostomy, ventilator support, G-tubes, and other life-sustaining medical equipment. As a Pediatric Home Care LPN, you will work independently in the home while supported by a 24/7 clinical team. You will implement physician-ordered plans of care, provide complex skilled nursing services, and partner closely with families to ensure optimal outcomes.

Responsibilities

• Implement and follow physician-prescribed plans of care
• Provide one-on-one nursing care to medically fragile pediatric patients
• Administer medications, and treatments as ordered
• Provide skilled care including (as applicable):
• Tracheostomy care
• Ventilator management
• G-tube/NG tube feedings
• Suctioning and airway management
• Oxygen therapy
• Seizure monitoring
• Wound care
• Catheter care

Assessment & Monitoring

• Perform comprehensive pediatric assessments
• Monitor respiratory status, neurological status, and overall condition
• Identify and report changes in patient status promptly
• Respond appropriately to emergencies within scope of practice

Documentation & Compliance

• Maintain accurate and timely electronic documentation
• Ensure compliance with Medicaid and state regulations
• Participate in care plan updates and coordination of care

Family Education & Support

• Educate and train caregivers on safe equipment use and daily care routines
• Provide health teaching and reinforcement of care techniques
• Support families in promoting independence and quality of life
