<Responsibilities>
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and specialized treatments such as botulinum toxin or dermal fillers.
- Assist with medical procedures such as catheterization, suturing, tube feeding, spinal taps, and specimen collection/processing.
- Maintain detailed medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks to ensure compliance with HIPAA standards.
- Monitor patients’ conditions closely in acute care settings such as ICU or Level I Trauma Centers, providing critical support in emergency medicine and trauma situations.
- Support discharge planning and case management efforts by coordinating post-acute care and collaborating with multidisciplinary teams.
- Educate patients and their families on health coaching topics including dementia care, Alzheimer's care, pain management, nutrition, and home & community care strategies.
<Skills>
- Proven experience working in diverse healthcare environments such as hospital medicine, nursing homes, hospice care, or outpatient clinics.
- Strong knowledge of medical terminology, anatomy, physiology, ICD-9/10 coding systems for accurate documentation and billing.
- Skilled in IV insertion, phlebotomy, venipuncture, catheter care, and sterile processing techniques.
- Familiarity with EMR/EHR systems like Epic or Cerner for efficient patient documentation.
- Ability to perform patient assessments with attention to detail in triage situations or urgent care settings.
- Experience managing patients with complex needs including infectious diseases, acute pain management, dementia or memory care cases.
