Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications, injections, IV infusions, and other treatments following established protocols and physician orders.
- Manage complex wound care, including dressing changes and infection control measures, ensuring optimal healing outcomes.
- Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, or Athenahealth for accurate documentation of patient care activities.
- Educate patients and their families on health management strategies, including medication administration, nutrition, and chronic disease management such as Alzheimer's or dementia care.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists like physical therapists or social workers.
- Conduct patient triage over the phone or in person to assess urgency levels and determine appropriate interventions or emergency responses.
Experience
- Valid Licensed Practical Nurse (LPN) license with current state certification.
- Prior experience in home health nursing or community-based care preferred; experience in medical-surgical units or hospitals is highly valuable.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Critical care experience including ICU or emergency medicine enhances your ability to handle acute situations confidently.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and medication administration protocols is essential.
- Experience working with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer's care is a plus.
- Familiarity with IV infusion techniques, catheterization procedures, tube feeding management, and ventilator support is advantageous.
