Licensed Practical Nurse (LPN)

3801 Grand is looking to add an exceptional LPN to our team to fill a new role created by the expansion of LPN capabilities in assisted living. This expanded role will be a great fit for the right individual with a deep passion for senior care.

Role Details:

- All roles at 3801 Grand are focused on great care for our residents - Nursing, Personal Care, Dining, Housekeeping & Laundry, Maintenance, Activities, Transportation - The LPN plays a critical role on the team to make the lives of our residents better.

- The role reports to and works closely with the department leader, the Director of Nursing. The LPN supports the DON through task-oriented nursing reviews/assessments, pharmacy/physician/family communication, and other regulated tasks.

- The LPN co-leads policies, training initiatives, the well-being of residents, and employee support in coordination with the DON and the organization as a whole.

Specific Responsibilities:

- The LPN and DON will equally split responsibilities for evening/weekend phone coverage for personal care staff questions/concerns. This very rarely results in the need for physical “on-call coverage”, but phone access for questions is a requirement of the role.

- The LPN will conduct most 90-day nursing reviews for residents, will perform additional nursing assessments as assigned, and will handle most pharmacy / physician’s office matters. Additional documentation / filing tasks will also be required.

- Responding to resident health concerns, being a secondary communication point for families, auditing meds and records, and representing the DON and organization are key responsibilities.

- Occasional coverage of medication pass / floor leadership coverage may be required when other coverage cannot be found. This is typically rare.

Summary:

- 3801 Grand wants to find the right individual for this role, and that could be a newly licensed LPN or an individual with fantastic experience. Starting wage ranges from $32/hr - $35/hr based on experience, and benefits include vacation and personal time PTO, 401k matching, health/dental/vision insurance, flex spending, and a $25/mo phone stipend.
