Position Summary
The Licensed Practical Nurse (LPN) delivers skilled nursing services to individuals of all ages (pediatric, adult and geriatric) receiving home and community-based services throughout South Carolina. Working under the supervision of a Registered Nurse (RN) and in accordance with physician orders, the LPN provides direct clinical care, administers medications and treatments, monitors health conditions, and responds to changes in status. The LPN supports individuals with Intellectual and Developmental Disabilities (IDD) and other medical needs, promoting safety, dignity, independence, and an optimal quality of life.
Position Scope
This position operates within the Nursing Department and supports participants in home or community-based settings. The LPN provides direct hands-on clinical care while collaborating with interdisciplinary teams to ensure compliance with care plans, regulatory standards, and OneWell policies.
The LPN functions within the scope of licensure under South Carolina law and does not independently practice beyond delegated authority.
Key Responsibilities
The Licensed Practical Nurse (LPN) delivers physician-directed nursing care by implementing and monitoring individualized care plans, administering prescribed medications and treatments, and performing clinical duties such as monitoring vital signs, providing wound care, supporting PEG/G-Tube or tracheostomy care (as applicable and trained), and observing and reporting changes in health status to the supervising RN. The LPN maintains accurate and timely documentation, supports participants with daily living needs when required, and collaborates effectively with families, caregivers, and healthcare providers. This role ensures compliance with all state and agency regulations and performs additional duties consistent with licensure and organizational needs.

Confidentiality
The LPN is required to maintain strict confidentiality of all participant information in compliance with HIPAA and OneWell policies. Protected Health Information (PHI) must only be accessed, discussed, or disclosed as necessary for treatment, payment, or healthcare operations.
Documentation Requirements
Timely completion of nursing notes
Medication Administration Records (MAR) maintenance
Incident reporting documentation
Treatment and wound care documentation
Compliance with electronic health record (EHR) systems
Accurate shift documentation
Certifications
Active South Carolina LPN License (required)
CPR/BLS Certification (required)
Current TB test or screening (required)
Additional certifications as required by specific case assignment
Education
Graduate of an accredited Licensed Practical Nursing (LPN) program
Experience
Minimum one (1) year of nursing experience required
Experience in home health, IDD services, or long-term care preferred
Experience with G-Tube, Trach, or Vent care strongly preferred (case dependent)
Technical Skills
Proficiency in medication administration
Basic wound care and infection control knowledge
Familiarity with electronic documentation systems
Ability to interpret physician orders
Basic computer and mobile device proficiency
Languages
English required Bilingual abilities preferred (not required)
Driver License
Valid driver’s license required. Reliable transportation required. Ability to travel to participant homes within assigned region.
Travel Requirement
This position requires local travel within South Carolina to participant homes and community settings. Travel percentage varies based on caseload and geographic assignment.
