Job Overview
We are seeking a compassionate and dedicated Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients across diverse settings, including inpatient, outpatient, hospice, and community care environments. Your expertise will help improve patient outcomes by offering emotional support, clinical counseling, and case management while ensuring compliance with healthcare regulations. Duties

  • Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to determine individual needs and develop personalized care plans
  • Provide clinical counseling and psychotherapy to individuals and groups, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy
  • Assist with crisis intervention efforts, managing urgent situations with sensitivity and professionalism
  • Coordinate care through case management activities, including intake procedures, medical documentation, and collaboration with multidisciplinary teams
  • Maintain accurate and compliant medical records in electronic medical record (EMR) systems while adhering to HIPAA privacy regulations
  • Support patients with specialized needs such as addiction counseling, Alzheimer's care, PTSD care, autism spectrum disorder interventions, dementia care, and working with individuals with developmental disabilities
  • Facilitate group therapy sessions and conduct behavioral health interventions tailored to diverse populations in inpatient, outpatient, hospice, or assisted living settings

Experience

  • Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant state or region
  • Proven experience in clinical counseling within hospital settings or community-based programs
  • Demonstrated expertise in child & family counseling, hospice care, geriatrics, pediatrics, or working with individuals with disabilities
  • Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and medical records management
  • Experience conducting diagnostic evaluations for mental health conditions such as PTSD or autism spectrum disorders
  • Knowledge of crisis management techniques and experience working in high-pressure environments requiring swift decision-making
  • Strong understanding of healthcare regulations including HIPAA compliance and data collection protocols
