Responsibilities:

1. Patient Assessment:

3. Patient Education:

- Provide ongoing education on chronic disease management, including medication adherence, lifestyle modifications, and preventive measures.

4. Remote Monitoring:

- Utilize telehealth tools and remote monitoring technologies to track and assess patients' health status and progress.

5. Behavioral Interventions:

- Implement behavioral interventions to support patients in adopting and sustaining positive health behaviors.

6. Medication Management:

- Monitor medication adherence, educate patients on prescribed medications, and collaborate with healthcare providers to optimize treatment plans.

7. Emotional Support:

- Offer emotional support and counseling to help patients cope with the emotional aspects of managing chronic conditions.

Qualifications:

- Licensed Vocational Nurse (LVN) license.
