Job Summary:
We are actively hiring licensed and credentialed LVN for PRN, part-time, or flexible work in the home health setting. This is an ideal opportunity for professionals seeking supplemental income, improved work-life balance, or a rewarding side hustle.

Key Responsibilities:
* Perform patient evaluations and implement individualized care plans
* Deliver direct care in the home setting
* Communicate and collaborate with interdisciplinary care teams
* Educate patients and families on condition management and self-care
* Document visits and maintain accurate clinical records
* Travel to patients' homes throughout the Bay Area

Qualifications:
* Current and active California LVN license
* Minimum of 1 year clinical experience (home health experience preferred but not required)
* Valid driver’s license, auto insurance, and reliable transportation
* Strong interpersonal, time management, and documentation skills
* Strong written and verbal communication skills
* Comfortable working independently in a home-based setting
