LMSW Palliative Care Social Worker

Description:

The LMSW Palliative Care Social Worker navigates health care systems, social services, and provides support that ensures the health and wellness for Snoqualmie Tribe Elders. Advises Tribal Elders program participants of information regarding resources that support Tribal Elders on aging related issues. Provide relevant services and work directly with Tribal Elders in the field.

Essential Duties:

  • Conduct assessments and home visits and develop care plans that support Tribal Elders and their needs. Identifies economic, social, cultural, nutritional, physical, and environmental factors which support or limit family or individual functioning.
  • Evaluate the effectiveness of the care plans and adjust as needed. Develop and implement individual care plans in partnership with elders and their families.
  • Connect Tribal Elders with essential community resources with face-to-face hands-on assistance with Tribal Elders in the field.
  • Share information with the families of Tribal Elders and Tribal staff on State or Federal resources that are beneficial to Tribal Elders.
  • Work closely with groups and individuals to help build and maintain positive relationships.
  • Address elder abuse, neglect, or exploitation concerns and work with appropriate authorities when necessary.
  • Provide culturally sensitive crisis intervention, grief support, and guidance
  • Provide advocacy and support with applications, referrals, and service coordination.
  • Connect elders with internal tribal programs and external agencies, including Medicare, Medicaid, Veterans’ services, and elder benefits .Can be Authorized Representatives on DSHS accounts if Elder’s legal consent is provided to the employee.
  • Educate Tribal Elders and their families on aging related issues.
  • Collaborate with medical professionals and other service providers.
  • Make appropriate recommendations and assist the supervisor with planning of department goals and objectives.
  • Maintain client records and documentation. Gather, compile, and analyze data and prepare reports.

Additional Duties:

  • Participate in department meetings and training as appropriate. Participate in continuing education as required and/or needed to fulfill job duties.

Requirements:

Position Requirements:

  • Strong understanding of Native culture, values, and intergenerational care practices.
  • Strong understanding of aging-related health conditions.
  • Knowledge of community resources for older adults.
  • Empathy and patience in dealing with Elderly clients.
  • Excellent interpersonal, communication, and organizational skills.
  • Proficiency in case management, documentation, and client confidentiality standards.
  • Ability to work independently and as part of a team.
  • Excellent customer service skills and written and verbal communication. Demonstrated ability to communicate effectively with diverse populations.
  • Effective problem solving and conflict resolution skills.
  • Ability to maintain confidentiality and use tact and discretion.
  • Proficiency with Microsoft Office programs.
  • Strong organizational skills and documentation skills with accuracy and attention to detail.

Education & Experience:

Education Required:

  • Master’s degree in social work, human services, gerontology, or a related field.

Experience:

  • Experience with case management software.
  • Minimum of 2 years of experience working with elders, preferably in a tribal or Indigenous community setting.

Certifications/Licenses:

  • Valid unrestricted Washington State driver’s license required. Qualified driving record required.
  • Current LMSW licensure as a Social Worker.

Other Requirements:

  • Upon hire, successfully complete and pass required training, such as defensive driving, mandatory reporting, and HIPAA compliance.
  • Current CPR/1st Aid card, TB immunization, and blood borne pathogens training preferred; required to obtain all within first six months of hire.
  • Must be able to pass a thorough pre-employment background investigation.

Work Environment:

Office and field-based work, including home visits and travel within the tribal community. May require evening or weekend work for elder events or emergencies. Must be able to lift up to 25 lbs. and assist with mobility needs on occasion.

The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.

Additional Information:

  • Compensation will be determined based on experience and starts at $88,726.03 per year.
  • This position is a full-time position and employment is at will.
