**Qualifications Required**

• Board Certification in Obstetrics and Gynecology (Board Eligible Status Not Accepted)

• Required

• Michigan Medical License and Controlled Substance Registration (CSR) at Time of Submission

• Required

• Basic Life Support (BLS) Certification

• Required

• Providers must be local to the area

• Required, with no exceptions

• A clean background is mandatory for expedited credentialing

• Required, with no exceptions
