Locum - Physician - Otolaryngology

Schedule / Availability:

• Dec 15–17 (specific dates)

• Potentially 2 weekends per month ongoing, pending approvals for additional coverage

• 24-hour call: 7:00 AM – 7:00 AM

Weekend / On-Call Requirements:

• Weekend and night coverage as needed

Required Procedures / Job Duties:

• Standard ENT coverage

• No rounding required

• Anesthesia support available

• EMR: EPIC

EMR Training Requirements:

• Pre-application and full application must be submitted to initiate “applicant in training” status for AD account creation

• Module completion required prior to virtual EMR training

• Virtual EMR training must be completed at least one week prior to first day

• Candidates must complete an EMR questionnaire at the time of submission including:

• Years of Epic experience

• Last date of Epic use (MM/YYYY)

• Last facility where Epic was used

• Provider photo for badge

• Time zone for scheduling virtual training

• Best phone number for call shifts

• Personal email for pre-application and program communications

Travel Preferences:

• Local / drive-in candidates highly preferred

Credentialing Information:

• Timeframe: less than a week for temps

• Credentialing with other facilities: No

• Requirements: Self-inquiry NPDB report, active CO license or IMLC

• Pre-application and full application must be submitted within 72 hours to allow AD account creation (up to 5 days) for EMR access

Requirements:

• Board Certified Otolaryngology – Head and Neck Surgery – [REQUIRED]

• Active CO License or IMLC – [REQUIRED]

• Active BLS – [REQUIRED]

• Active CO DEA – [HIGHLY PREFERRED] (must maintain DEA for assignment duration)

• Candidate availability for dates detailed in job description – [REQUIRED]

• Number of shifts per month provider can offer – [REQUIRED]

• Acknowledgment of EMR training requirements – [REQUIRED]

• Understanding of preferred lodging requirement and travel policy – [REQUIRED]
