Long Term Care Supervisor

Summary

The hiring salary for this position is $66,486.38 annually.

Charles County Government is seeking an experienced Long Term Care Supervisor to perform long-term care aging services to eligible clients and their families, as well as related administrative support. This position will provide administration including annual budget management, service and guidance for the multiple state and federally funded programs for eligible older adults and their family caregivers. Supervisor will assist with planning and coordination of long term care services, provide information on community resources, conduct any required screening (including but not limited to a level one screen), complete eligibility applications, provide case management services and prepares local, state, and federal reports for the associated programs.

Essential Job Functions

  • Administration of Adult Public Guardianship Program and coordination with staff to ensure the completion of client visits.
  • Assists in planning, coordinating, and implementing long term care programs as assigned. Including preparing authorization letters, submitting financial reimbursement forms for enrolled eligible clients, and managing fee for service billing for contractual services.
  • Maintains records of fund allocation and authorizes funds within program guidelines.
  • Maintains client records, statistical data, prepares programmatic and financial reports and verifies invoices for payment processing.
  • Administers and monitors monthly, quarterly and annual budgets for state and federal grant programs.
  • Maintains confidential client files, statistical data entry, spreadsheets, and wait lists for all programs. Completes all program related reports to align with Federal and State program guidelines, as well as local performance measures.
  • Coordinates the Volunteer Representative Payee program.
  • Serves as lead staff for inter-agency Joint Care Plan meetings and may represent Division at Interdisciplinary Department Team meetings as needed.
  • Conducts or supports Maryland Access Point intake, initial assessment and performs ongoing assessments to determine continued client eligibility for benefits and services. Develops initial and updated service plans as appropriate.
  • Provides telephone and in-person assistance involving all aging programs, including information to senior citizens, families, Long Term Care facilities, agencies, and community organizations.
  • Facilitates monthly caregivers support group and disseminates information of interest to caregivers regularly via e-mail.
  • Serves on councils, committees and boards as requested.
  • Serves on the planning committee for Southern Maryland Caregivers Conference and facilitates Charles County service as a host site on assigned years.
  • Provides support assistance for other Long Term Care programs and Aging/Human Services programs.
  • Supervises, evaluates, and recommends personnel actions for assigned staff in accordance with the County Personnel policies and procedures.
  • Maintains the Respite Registry and other in-home care resource information.
Performs other related job duties as assigned.

Qualifications, Knowledge, Skills, and Abilities

Education and Experience:
Bachelor’s degree in human services, geriatrics, social work, behavioral science, health care, or related field. Three (3) years of progressive experience in human services or aging programs; or an equivalent combination of education, experience, and training.
Licenses or Certifications:
Must possess a valid driver's license. Possessing current RN license or LBSW license is preferred but not required.
Special Requirements/Qualifications:
Work is subject to frequent interruptions.
Subject to work beyond the normal scheduled hours of work.
Subject to a Criminal Background check.
May be required to make home visits and drive to various locations in the county.
Knowledge, Skills and Abilities:
  • Knowledge of community resources and public benefit programs.
  • Knowledge of health conditions and sociology of the older adult population and family caregivers.
  • Knowledge of effective methods and techniques of counseling.
  • Knowledge of elderly population.
  • Knowledge of PC operations.
  • Ability to counsel older persons.
  • Ability to investigate complaints regarding care and other matters and take or recommend appropriate actions to resolve such matters.
  • Ability to interpret state and federal regulations.
  • Ability to communicate effectively orally and in writing.
  • Ability to establish and maintain effective working relationships with others encountered in the work.
  • Ability to initiate own daily assignments, follows through on matters, and uses independent judgment and training to take appropriate actions to deal with standard recurring situations.

PHYSICAL DEMANDS
The work is sedentary with frequent periods of walking and standing. Typical positions require workers to lift and carry up to 50 pounds; climb stairs; bend and crouch; reach, hold, grasp, and turn objects; and use fingers to operate computer or typewriter keyboards; and the ability to speak normally, to use normal or aided vision and hearing.
WORK ENVIRONMENT
Principal duties of this job are performed in a general office environment and that occasional field work, including home visits, is required.
