Compensation: $105,000 to $120,000 annually + autonomy + growth opportunity.

Position Summary:

This is a career-elevating opportunity for a Nurse Practitioner who wants more autonomy, more variety, and more meaningful patient interaction. As a Wound Care focused NP, you will manage complex wounds, lead care plans, and become a trusted clinical presence within your communities. You will step into a role that blends advanced wound care, mobile primary care, and long-term patient relationships, giving you the chance to grow both clinically and professionally.

Responsibilities:

Provide on-site rounding, primary care, and advanced wound care services.
Manage complex wounds including pressure injuries, ulcers, and post-acute cases.
Drive chronic disease management and preventative care strategies.
Collaborate with physicians, nursing staff, and interdisciplinary teams.
Reduce hospitalizations through proactive clinical interventions.
Communicate with families, caregivers, and facility leadership.
Complete documentation, medication management, and lab coordination.

Qualifications:

Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
Experience in wound care, geriatrics, primary care, or post-acute care preferred.
Valid driver’s license and ability to travel between communities.
Strong ability to work independently in a field-based environment.
Passion for delivering high-quality, patient-centered senior care.
