Position Summary:
This is a career-elevating opportunity for a Nurse Practitioner who wants more autonomy, more variety, and more meaningful patient interaction. As a Wound Care focused NP, you will manage complex wounds, lead care plans, and become a trusted clinical presence within your communities. You will step into a role that blends advanced wound care, mobile primary care, and long-term patient relationships, giving you the chance to grow both clinically and professionally.
Responsibilities:
- Provide on-site rounding, primary care, and advanced wound care services.
- Manage complex wounds including pressure injuries, ulcers, and post-acute cases.
- Drive chronic disease management and preventative care strategies.
- Collaborate with physicians, nursing staff, and interdisciplinary teams.
- Reduce hospitalizations through proactive clinical interventions.
- Communicate with families, caregivers, and facility leadership.
- Complete documentation, medication management, and lab coordination.
Qualifications:
- Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
- Experience in wound care, geriatrics, primary care, or post-acute care preferred.
- Valid driver’s license and ability to travel between communities.
- Strong ability to work independently in a field-based environment.
- Passion for delivering high-quality, patient-centered senior care.
