LPN (Contract to Perm) – NY 14456

• Contract Length: 13 Weeks (05/12/2026 – 08/15/2026)

• Schedule: Full-Time, 5x8-hour evening shifts

• Shift Time: Evenings (e.g., 3:00 PM – 11:30 PM)

• Hours: 40 hrs/week

• Pay: $1,063 – $1,204/week

• Requirements: Every Other Weekend (EOW) + Holidays required

Position Summary & Responsibilities

• Provide skilled nursing care to adult/geriatric residents under RN supervision

• Assess, monitor, and report physical/psychosocial status changes

• Administer treatments/medications and support ADLs per care plan

• Contribute to and update individualized care plans with interdisciplinary team

• Maintain accurate documentation and ensure patient safety/compliance

Requirements

• Active NY LPN Licensure

• BLS Certification (AHA or RQI required)

• Minimum 1 year LPN experience (preferably in LTC setting)

• Ability to care for adult and geriatric populations

• High School Diploma or GED (plus accredited LPN program)

• Willingness to work in environment with potential exposure to blood/body fluids.
