LPN Coordinator/Dispatcher

Trustbridge Hospice, part of Empath Health, is seeking a caring and motivated LPN Coordinator to join our team in West Palm Beach. This role is ideal for an LPN who is looking for a consistent weekend-only schedule that allows for flexibility during the week while making a meaningful impact. The schedule will be: every Friday/Saturday/Sunday 6:00am – 6:30pm. The ideal candidate will play a vital role in ensuring seamless coordination of care by supporting staff visits and collaborating with interdisciplinary team members as well as dispatching emergent visits. You’ll act as a central connector—helping teams respond efficiently and compassionately to patient and family needs.

What you'll Do

  • Coordinate with Managers and care team members to support patient and family direct and indirect care needs
  • Provide quality professional intervention guided by established policies and protocols
  • Collaborate closely with management, physicians, and field staff using a teamwork-driven approach
  • Prioritize patient comfort and family-centered care in every interaction
  • Deliver consistently personalized, sincere, and engaging service to patients and families
  • Support and promote complete patient and family satisfaction

What You'll Need

  • Current license to practice as a Licensed Practical Nurse in the State of Florida.
  • Current Basic Life Support Certification.
  • Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
  • Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
  • Basic computer skills and EMR software experience a plus.
