What Makes Home Care Different?
As an LPN on our Constitution team, you will:
- Deliver 1:1 skilled nursing care
- Work collaboratively with RN Case Managers
- Practice at the top of your license
- Provide complex clinical treatments
- Support patients recovering from surgery, illness, or chronic disease exacerbations
You won’t “manage” patients — you’ll focus on delivering high-quality, hands-on clinical care and education that prevents hospital readmissions and improves lives.
Primary coverage will be in:
• Malden • Melrose • Everett
Team territories also include:
Arlington • Medford • Sommerville
While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible!
Schedule & Workload
- Full-time (40 hours) preferred
- 32-hour and 36-hour options available
- Monday–Friday schedule
- Average of 6–7 visits per day
- Dedicated time for documentation, care coordination, and chart review

Job Summary
Provide quality nursing care to patients in various settings in the community. Patient care is delivered within MGB Home Care's philosophy, policy, and standards of community health practice. In collaboration with the RN Case Manager this position will provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures and standards of practice.
- LPN professional license in Massachusetts
- Minimum 1-year previous nursing in acute care or sub-acute care setting required
- Home care experience preferred
- Effective written, verbal and electronic communications skills
- Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred
- Ability to communicate effectively in writing, verbally and electronically
- Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population
- Strong interpersonal skills with ability to work with varied population
- Ability to work as a member of an interdisciplinary team
- Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
- Travel required in local geographic area
