LPN Home Health Intake Coordinator

Position Summary:

The regional leader in Mobile Wound Care is looking for a highly skilled Patient Intake Coordinator. The Patient Intake Coordinator plays a critical role in the day-to-day operations of the healthcare facility by working with our partner organizations to ensure our patients are receiving the best possible care. These individual supports patient care coordination, and provides exceptional customer service to patients, families, and medical staff, and our partner organizations.

Key Responsibilities:

• Receive and input incoming patient inquiries via phone, email, or in person into the referral tracker and into the EMR.
• Conduct initial assessments to determine patients' needs and eligibility for services.
• Coordinate appointments and schedule patient discussions with appropriate staff.
• Maintain accurate records of patient information and interactions in the database.
• Collaborate with internal teams to ensure seamless coordination of services for patients.
• Provide information, resources, and referrals to patients, home health, case managers, clinical staff, account executives, and assisted living/long term facilities as needed.
• Monitor and track client progress and follow up as necessary.
• Adhere to HIPAA regulations and maintain patient confidentiality at all times.
• Other duties as assigned

Qualifications:

Education & Experience:

• Licensed Practical Nurse (LPN)

Skills:

• Proficient in using medical scheduling software.
• Strong communication, customer service, and interpersonal skills
• Ability to multitask and remain calm in a fast-paced environment
• Detail-oriented with strong organizational and time-management skills

Working Conditions:

• Office-based role with prolonged periods of sitting and computer use

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
