Join Anpex Health and be part of an innovative, patient-centered team delivering high-quality care directly in patients’ homes while collaborating with a Nurse Practitioner-led virtual care model.
Conduct comprehensive in-home patient assessments, including vital signs, medication reconciliation, and focused physical observations
Assist with and facilitate Nurse Practitioner (NP)-led telehealth visits using mobile devices and telehealth platforms
Communicate clinical findings clearly and accurately to the NP and interdisciplinary care team
Complete timely and accurate documentation in the electronic medical record (EMR)
Identify and report changes in patient condition or safety concerns
Provide patient education and reinforce care plans as directed by the NP
Support care coordination and continuity between in-home visits and virtual care
Ensure compliance with clinical protocols, company policies, and HIPAA regulations
Active Licensed Practical Nurse (LPN) license in good standing
Experience in home care, geriatrics, or primary care preferred, but not required
Training provided for telehealth workflows and in-home assessment model
