LPN Home Visits

Position Overview:
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team in a community-based role. This position involves conducting in-home patient assessments and supporting Nurse Practitioner (NP)-led telehealth visits. The ideal candidate is comfortable working independently in patients’ homes while collaborating closely with a virtual clinical team.

Key Responsibilities:

• Conduct comprehensive in-home patient assessments, including vital signs, medication reconciliation, and focused physical observations

• Assist with setting up and facilitating NP-led televisits using mobile devices or telehealth platforms

• Communicate patient findings clearly and accurately to the NP and care team

• Document assessments and interventions in the electronic medical record in a timely manner

• Identify and report changes in patient condition or safety concerns

• Provide basic patient education and reinforce care plans as directed by the NP

• Ensure compliance with all clinical protocols, policies, and HIPAA regulations

Qualifications:

• Active Licensed Practical Nurse (LPN) license in good standing

• Prior clinical experience in home care, geriatrics, or primary care is preferred, but not required; training will be provided

• Strong assessment and communication skills

• Comfortable with technology, including tablets and telehealth platforms

• Ability to work independently and manage time effectively

• Reliable transportation and willingness to travel to patient homes

Preferred Qualifications:

• Experience with home visits or community-based care

• Experience with electronic medical records (EMR) systems
