Holland Home, Michigan’s largest non-profit senior care provider, is seeking a compassionate and skilled LPN Response Nurse to join our Raybrook Assisted Living team. This role is perfect for an LPN who thrives in a leadership position and values person-centered care.

As a Response Nurse, you’ll oversee the care team, ensure high-quality care for residents, and respond to medical needs during your shift. You’ll also serve as a resource and mentor for Resident Assistants, promoting a supportive and collaborative environment.

What You’ll Do

Provide direct nursing care, including medication administration, assessments, and treatments
Respond to emergencies and initiate appropriate care
Supervise and coach care staff, ensuring compliance with standards and regulations
Build trusting relationships with residents and families, offering emotional support
Act as a liaison between residents and physicians, managing orders and communication
Maintain accurate documentation and participate in interdisciplinary care planning

Position Details

Location: 2121 Raybrook St SE, Grand Rapids, MI 49546
Schedule:
- 36 hours per pay period
- Monday 6:30pm-7am
- Friday 1pm-7pm

Qualifications

Current MI LPN license
CPR certification required
Experience with geriatrics preferred
Basic computer skills for charting in PCC
Strong interpersonal and leadership skills
