Licensed Practical Nurse (LPN) – Sub-Acute Rehab – 13 Week Contract | 3445547

We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week contract assignment in a Sub-Acute Rehabilitation or LTACH setting. This role is ideal for LPNs comfortable managing medically complex patients transitioning from hospital to post-acute care.

Key Responsibilities

• Provide direct patient care to sub-acute and rehabilitation patients
• Monitor vital signs and patient status, reporting changes promptly
• Administer medications and treatments as ordered
• Assist with wound care, IV therapy (as permitted by scope), and rehabilitation support
• Document patient care accurately and timely
• Collaborate with RNs, physicians, and interdisciplinary care teams
• Maintain compliance with facility and regulatory standards

Qualifications

• Active LPN license (state or compact as applicable)
• Minimum 1–2 years of recent Sub-Acute Rehab, Skilled Nursing, or LTACH experience required
• Experience caring for medically complex patients required
• Strong clinical assessment and documentation skills
• Ability to function effectively in a fast-paced post-acute environment

Contract Details

• Assignment Length: 13 weeks
• Schedule: 36 hours per week (3×12-hour shifts)
• Day and Night shifts available
• Employment Type: Contract (not permanent)
