WoundWorx is a fast-growing mobile wound care practice serving patients across Maryland, DC, and Northern Virginia. We bring expert wound care directly to patients in their homes, assisted living facilities, and skilled nursing facilities. Led by Dr. Joanna King, DNP, FNP-BC, CWCN-AP, we specialize in advanced wound management including debridement, skin substitutes, negative pressure wound therapy, vascular diagnostics, and therapeutic ultrasound.

We are launching a Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) program integrated with our CharmHealth EHR, and we need a clinical RN who can work in the field two days per week and manage our RPM/CCM program remotely three days per week.

What You'll Do

Field Days — Clinical Wound Care (2 days/week, ~40% of role):

• Perform wound assessments, measurements, and photo documentation during mobile home visits alongside Dr. King
• Assist with debridement, NPWT changes, skin substitute applications, and UltraMIST treatments
• Perform ankle-brachial index (ABI) testing using MESI mTABLET Smart ABI devices
• Enroll new patients into RPM — set up FDA-cleared devices (BP cuffs, scales, pulse oximeters, CGM) and educate patients on use
• Document all visits in CharmHealth EHR with wound photos, measurements, and treatment notes
• See 8-12 patients per field day across assigned geographic territory

Remote Days — RPM & CCM Program Management (3 days/week, ~60% of role):

• Monitor incoming vital sign alerts and device readings from the CCN Health RPM dashboard
• Triage out-of-range readings and escalate to Dr. King or referring providers as needed
• Perform monthly CCM check-in calls for patients with 2+ chronic conditions (20-30 min per call)
• Update care plans, perform medication reconciliation, and document clinical time for Medicare billing
• Ensure 16+ days of device readings per patient per month for CPT 99454 compliance
• Track RPM/CCM time logs for billing — CPT 99457 (first 20 min), 99458 (each additional 20 min), 99490 (CCM 20 min), 99439 (CCM additional 20 min)
• Coordinate with scheduling VA on patient appointments and device deliveries
• Participate in weekly clinical huddles via video

Team Coordination (ongoing):

• Communicate with office manager, billing team, and NP on patient status changes
• Collaborate with Virtual RN (Tissue Analytics/CCM calls) on shared patient panels
• Support clinical quality metrics and wound healing outcome tracking

What You Bring

Required:

• Active Maryland LPN/RN license (compact/multistate preferred)
• 2+ years clinical nursing experience
• Reliable personal vehicle and valid driver's license for 2 field days per week (mileage reimbursed)
• Comfort with both mobile field nursing AND remote clinical work
• Strong computer skills — you'll work in CharmHealth EHR, CCN Health dashboard, and Spruce Health messaging daily
• Ability to work independently and manage your own remote schedule
• BLS/CPR certification
• Quiet, HIPAA-compliant home workspace for remote days

Preferred (we'll train the right person):

• Wound care experience (home health, SNF, or outpatient wound center)
• Experience with RPM devices or remote monitoring programs
• Experience with CharmHealth, PracticeGen, or CollaborateMD
• WCC, WOCN, or wound care certification
• Knowledge of Medicare billing (CPT codes 93922, 99453-99458, 99490)
• Experience with vascular diagnostics (ABI/TBI)
• Experience with CCM or care coordination programs
• Bilingual (Spanish) a plus

About Our Team

We're a growing team that includes clinical staff, billing specialists, virtual nurses, and operations support. You'll work directly with Dr. Joanna King in the field (2 days/week) and collaborate remotely with our Operations Manager, RCM Lead, Virtual RN team, and scheduling VA. We use CharmHealth for EHR, Spruce Health for clinical communications, and are integrating CCN Health for remote monitoring.
