Role Summary:

This is an exciting opportunity for a Licensed Vocational Nurse (LVN) to join a collaborative care team that supports adults with complex medical and mental health needs—including individuals experiencing homelessness and substance use disorders. Duties:

Direct Nursing Care & Care Coordination

  • Provide essential nursing services, including administering medications and injections, and monitoring clients with severe mental illness and co-occurring disorders.

  • Coordinate primary and specialty medical appointments to ensure timely and appropriate access to care.

  • Maintain accurate medication records and work with pharmacies to ensure prescriptions are filled and delivered correctly.

  • Offer education to clients to promote medication adherence, improve health literacy, and encourage overall wellness.

  • Work as a key member of an interdisciplinary team to deliver trauma-informed, client-centered care.

  • Communicate effectively with both internal team members and external healthcare providers to ensure seamless coordination of services.

  • Provide crisis intervention as needed, including triaging clients to emergency services.

  • Actively contribute to team meetings and one-on-one supervision sessions to support continuous learning and care quality.

  • Complete accurate, timely documentation in the electronic health record (EHR) system.

  • Remain informed about public health benefits and assist clients in accessing and navigating healthcare resources.

  • Adhere strictly to HIPAA regulations and confidentiality standards.

Required Qualifications:

  • Current and valid California LVN license

  • Experience in health sciences, social services, mental health, or related field

  • 1–3 years of clinical experience working with individuals with severe mental illness or other complex needs (preferred)

  • Strong understanding of HIPAA and patient confidentiality standards

  • Proficiency with Electronic Health Records (EHR) systems

  • Ability to remain calm, compassionate, and effective in high-stress situations or when engaging with challenging client behaviors

  • Valid California driver’s license with a clean driving record and ability to be insured

  • Live Scan clearance

  • CPR certification

Desired Qualifications:

  • Experience working in trauma-informed or harm-reduction–based environments

  • Bilingual skills or cultural competency working with diverse populations

  • Passion for community health, behavioral health, and serving underserved populations
