Position Summary:
This Community-Based Physician role offers an opportunity to build continuity of care across senior living environments. You’ll round within assigned communities, conduct comprehensive medical evaluations, manage chronic illnesses, and work alongside experienced nurse practitioners and physician assistants who value mentorship and collaboration.

Key Responsibilities:

• Provide on-site medical rounds and follow-up visits for assigned residents
• Lead care planning discussions with families and facility leadership
• Manage chronic diseases, wounds, and acute medical concerns
• Document care accurately and efficiently within EMR systems
• Champion TCP’s approach of preventative, community-integrated care

Qualifications:

• Active Minnesota MD or DO license in good standing
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics
• Strong background in primary or post-acute care
• Excellent communication, empathy, and problem-solving skills
• Valid driver’s license and vehicle
