MDS Coordinator

Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing the Minimum Data Set (MDS) assessments and ensuring compliance with regulatory requirements. This position requires a strong background in medical management, critical care, and health information management, along with excellent communication and organizational skills.

Responsibilities

• Conduct comprehensive assessments of residents to complete MDS documentation accurately and timely.
• Collaborate with interdisciplinary teams to develop individualized care plans based on assessment data.
• Ensure compliance with federal and state regulations regarding MDS submissions and documentation.
• Provide training and guidance to nursing staff on MDS processes and documentation requirements.
• Monitor resident outcomes and implement quality improvement initiatives as necessary.
• Maintain up-to-date knowledge of changes in regulations related to MDS and healthcare documentation.

Requirements

• A valid nursing license is required; experience in skilled nursing care is preferred.
• Proficiency in PCC or similar electronic health record systems is highly desirable.
• Strong background in medical management, including home care experience.
• Excellent communication skills, both verbal and written, with the ability to collaborate effectively within a team environment.
• Attention to detail and strong organizational skills are essential for managing multiple tasks efficiently.
• Previous experience in medical documentation is an asset.
