A well-established Skilled Nursing Facility in Westchester County is seeking an experienced MDS Coordinator with proven SNF experience and strong regulatory knowledge. Skilled Nursing Facility experience is required.

Position Overview

The MDS Coordinator is responsible for accurate and timely completion of MDS assessments in accordance with CMS guidelines and New York State regulations. This role directly impacts reimbursement, survey performance, and overall regulatory compliance.

Requirements (Non-Negotiable)

• Active NY RN license required
• Minimum 2+ years recent Skilled Nursing Facility experience
• Minimum 1+ year MDS experience in a SNF setting
• Strong working knowledge of RAI process and PPS
• Experience with Medicare Part A and managed care documentation
• Proven understanding of NY DOH and CMS survey expectations

Key Responsibilities

• Complete and submit accurate MDS assessments in accordance with federal and NYS regulations
• Ensure timely transmission and compliance with CMS guidelines
• Collaborate with interdisciplinary team to support accurate coding and care planning
• Monitor PDPM components and reimbursement impact
• Prepare documentation to support survey readiness
• Participate in quality assurance and compliance initiatives
