MDS Coordinator

Overview
We are seeking a highly motivated and detail-oriented MDS Coordinator to join our dynamic healthcare team. In this vital role, you will lead the comprehensive assessment and documentation process for residents in skilled nursing facilities, ensuring compliance with federal and state regulations. Your expertise will support optimal patient care, accurate reimbursement, and quality improvement initiatives. Responsibilities

  • Conduct thorough Minimum Data Set (MDS) assessments for residents, ensuring accuracy and completeness in accordance with CMS (Centers for Medicare & Medicaid Services) guidelines.
  • Collaborate with interdisciplinary teams including nursing, therapy, social services, and medical staff to gather pertinent clinical information.
  • Review medical records, clinical documentation, and care plans to verify data integrity and compliance with HIPAA regulations.
  • Ensure timely completion of MDS schedules, including scheduled updates and quarterly reviews, to support appropriate reimbursement levels.
  • Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document assessments and maintain accurate medical records.
  • Stay current with evolving regulations related to Medicare, Medicaid, ICD-10/ICD-9 coding standards, DRG (Diagnosis-Related Group) classifications, and NCQA (National Committee for Quality Assurance) standards.
  • Assist in discharge planning, case management, and utilization review processes to optimize patient care transitions across inpatient, outpatient, hospice, or home care settings.
  • Support clinical documentation improvement initiatives by reviewing medical terminology and anatomy/physiology details for accuracy.
  • Participate in staff training on MDS processes, coding updates including CPT (Current Procedural Terminology), ICD coding systems, and healthcare compliance protocols.

Qualifications

  • Proven experience with MDS coordination within skilled nursing facilities or long-term care environments.
  • Strong knowledge of Epic EMR systems or comparable EHR platforms such as Athenahealth or eClinicalWorks.
  • Familiarity with managed care practices, Medicare regulations, ICD-10/ICD-9 coding standards, CPT coding, DRG classifications, and utilization management principles.
  • Critical care experience or ICU background is highly desirable; experience in hospital settings including Level I Trauma Centers or Level II trauma centers is a plus.
  • Excellent understanding of medical terminology, anatomy, physiology, and clinical documentation review techniques.
  • Nursing licensure or relevant healthcare certifications are preferred; experience in nursing home settings or outpatient clinics is advantageous.
  • Ability to interpret complex medical records accurately while maintaining HIPAA compliance.
