 Monday – Friday | 8:00 AM – 5:00 PM

 Starting Pay: $16/hour + Benefits

We are seeking a compassionate CCM Care Coach to join our growing healthcare team in San Antonio. This role supports patients with chronic conditions through monthly phone check-ins, care coordination, patient education, and documentation in the EMR system. The goal is to help patients stay engaged in their care plans and improve health outcomes.

Responsibilities:

• Conduct monthly check-in calls with CCM patients

• Assist patients in understanding and following care plans

• Coordinate with providers and care teams

• Document patient interactions in the EMR

• Help identify barriers to care and connect patients with resources

Qualifications:

• Healthcare, care coordination, or patient support experience preferred

• Strong communication and organizational skills

• Comfortable speaking with patients over the phone

• Basic computer/EMR skills required

• Bilingual (English/Spanish) is a plus
