Position Overview
The Virtual Care Support Services Medical Assistant supports virtual care delivery, Remote Patient Monitoring (RPM), and Quality Department initiatives, with a primary focus on patients with cardiovascular conditions. This role is critical to patient engagement, proactive monitoring, and care coordination and requires extensive phone-based communication with patients and caregivers.
The ideal candidate has prior experience in cardiovascular care, is comfortable working in a fast-paced, metrics-driven environment, and demonstrates strong clinical judgment within defined protocols.
Key Responsibilities
- Perform outbound and inbound phone-based patient outreach for RPM and chronic cardiovascular care programs
- Monitor and review RPM data (e.g., blood pressure, heart rate, weight) and identify trends or abnormal values
- Escalate clinical concerns to the Advanced Practice Providers (APPs) or physicians per established cardiovascular care protocols
- Educate patients on cardiovascular disease management, medication adherence, lifestyle modifications, and RPM device usage
- Conduct routine follow-ups to assess symptoms, adherence, and care plan effectiveness
- Document all patient interactions, monitoring data, and care coordination activities accurately in the EHR and RPM Platform.
- Collaborate with physicians, APPs, care managers, and quality teams to ensure coordinated care delivery
- Assist with Quality Department programs, including care gap outreach, quality measure support, and data collection
- Provide education and support to caregivers to enhance chronic cardiovascular disease management
- Maintain compliance with HIPAA, RPM billing workflows, and organizational policies
Required Qualifications
- High school diploma or equivalent required
- Completion of an accredited Medical Assistant program required.
- A minimum of 5 years of experience as a medical assistant is required
- Minimum 2-3 years of hands-on experience in a cardiovascular or cardiology care setting (REQUIRED)
- Strong understanding of cardiovascular conditions such as hypertension, heart failure, coronary artery disease, and arrhythmias
- Proficiency with Electronic Health Records (EHR) and telehealth or RPM platforms
- Excellent verbal communication skills with comfort managing high-volume phone outreach
- Knowledge of HIPAA, healthcare compliance, and clinical documentation standards
Preferred Qualifications
- Experience in Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM)
- Experience supporting quality programs, value-based care, or population health initiatives
