Job Overview
We are seeking a highly organised and empathetic Medical Chronic Care Management Assistant to join our team. This is a part-time, fully remote position. The successful candidate will play a crucial role in providing administrative and coordination support for our patients receiving chronic care. You will be instrumental in ensuring our patients and their families receive seamless, compassionate, and efficient service. The offered compensation for this role is between $28 - $35 USD per hour, dependent on experience.

Responsibilities
* Provide comprehensive administrative support to the chronic care management team to ensure smooth day-to-day operations.
* Act as a key point of contact for patients and their families, handling enquiries with professionalism and empathy.
* Coordinate and schedule patient appointments, follow-ups, and consultations with internal and external healthcare providers.
* Maintain and update electronic patient records with a high degree of accuracy and confidentiality.
* Assist clinical staff in the development and implementation of patient care plans.
* Liaise with healthcare professionals, pharmacies, and other third-party services to ensure integrated and effective patient care.
* Manage sensitive information with discretion, adhering to all data protection and patient confidentiality standards.

Qualifications
* Proven experience in a healthcare administration, medical assistant, or patient coordination role.
* Exceptional communication and interpersonal skills, with a clear ability to show empathy and compassion.
* Strong organisational and time-management skills, with the capacity to prioritise tasks effectively in an independent, remote setting.
* Proficiency in using electronic health record (EHR) systems and standard office software.
* A solid understanding of the importance of patient confidentiality.
* Self-motivated and able to work with minimal supervision.
* Previous experience in a hospice, palliative care, or chronic care environment is highly desirable.
* Familiarity with medical terminology would be a strong advantage.
