Our Concierge Team consists of three specialized functions — Facility Concierge, Pharmacy Concierge, and Clinical Concierge — working together to eliminate care silos, streamline communication, and deliver safe, timely, and compassionate support to Assisted Living Facilities (ALFs) across the U.S.

Role Overview
We are seeking motivated healthcare professionals with strong communication and organizational skills to join our AllCare Facility Concierge Team.
As a Facility Concierge Specialist, you will serve as the operational bridge between assisted living facilities, physicians, and pharmacies. You’ll manage patient onboarding, perform medication reconciliation, coordinate dispatch and deliveries, and maintain precise clinical documentation to ensure residents receive exceptional care.

What You'll do
- Facilitate patient onboarding and enrollment in EMR systems.
- Collect, reconcile, and validate medication histories and documentation from facilities and prescribers.
- Perform medication reconciliation during admission, monthly cycles, condition changes, and discharge or transfer.
- Review census updates, prepare cycle fill lists, and ensure MAR/eMAR accuracy across all patient records.
- Monitor incoming orders, verify patient and facility details, assign deliveries, and track completion status.
- Act as the primary point of contact for facilities, providers, and pharmacies to resolve discrepancies and handle urgent requests.
- Maintain HIPAA-compliant documentation and uphold strict accuracy and quality standards.
Requirements:
Qualifications
- Degree or certification in pharmacy, nursing, or another medical field.
- 2+ years of experience in healthcare coordination, pharmacy operations, or clinical support.
- Excellent English communication skills, both written and verbal.
- Strong attention to detail and accuracy in handling medical records.
- High energy and comfort working in a fast-paced, dynamic environment.
- Experience collaborating with U.S. healthcare providers, pharmacies, or ALFs (preferred).
- Familiarity with EMR/EHR systems, MAR/eMAR platforms, or pharmacy management systems (e.g., PioneerRx) (preferred).
- Understanding of HIPAA compliance standards (preferred).
