Medical Coordinator, Behavioral Health

We are seeking a Medical Coordinator, Behavioral Health to play a key role in supporting our highly regarded Behavioral Health team of physicians, nurse practitioners, and physician assistants. Essential Duties and Responsibilities

  • Obtain daily nursing reports regarding acute patients and prescription refill needs.
  • Manage and schedule provider visits for physicians and advanced practice providers on a daily basis, including scheduling visits in accordance with Medicare requirements and Solomon Medical guidelines.
  • Enter new patient demographic and insurance information into the EMR.
  • Initiate administrative tasks including implementing new facilities, onboarding providers, and creating monthly schedules.
  • Serve as a liaison between Solomon Medical’s billing company and facilities for insurance-related inquiries.
  • Serve as a liaison between facility administrators, nursing staff, and Solomon Medical clinicians to help ensure smooth operations.
  • Obtain reports including DME, insurance verification, prior authorizations, labs, and radiology for provider review and signature.
  • For new admissions, obtain discharge summaries and create new patient encounters within the EMR.
  • Transcribe key discharge summary information, including hospital course, updated medications, allergies, past medical history, past surgical history, and social history.
  • Maintain professionalism, business etiquette, and adherence to HIPAA standards as they relate to patient information.

Behavioral Health-Specific Responsibilities

  • Serve as the primary coordinator for the Behavioral Health team, supporting psychiatrists, nurse practitioners, and physician assistants across assigned facilities.
  • Coordinate provider schedules, psychiatric visits, and communication workflows to ensure timely follow-up and appropriate coverage.
  • Facilitate communication among behavioral health providers, facility nursing teams, social workers, pharmacy partners, and other relevant care team members.
  • Support coordination of behavioral health care plans, specialty psychiatry referrals, prior authorizations, and psychotropic medication-related workflows.
  • Monitor psychotropic medication use to support compliance with CMS F605 regulations, including tracking Gradual Dose Reduction (GDR) eligibility, coordinating provider review, ensuring documentation of clinical rationale when GDR is contraindicated, and confirming required regulatory documentation is completed and maintained.
  • Assist in tracking behavioral health quality metrics, including psychotropic utilization trends, GDR completion rates, behavioral incident follow-up, and compliance-related trends.
  • Review psychiatric encounter documentation for completeness related to psychotropic justification, target symptoms, GDR documentation, and required regulatory language prior to submission, when applicable.
  • Maintain and oversee standardized crisis escalation workflows for situations such as suicidal ideation, aggression, and behavioral emergencies, ensuring timely provider notification and proper documentation.
  • Manage urgent behavioral health follow-up needs and escalate concerns to clinical leadership when appropriate.
  • Maintain familiarity with behavioral health terminology, psychotropic medication monitoring, safety planning, and crisis response workflows.

Education and Experience

  • Bachelor’s degree in healthcare administration, nursing, or a related field preferred.
  • Previous SNF experience or experience in a medical or healthcare setting strongly preferred.
  • Familiarity with electronic medical record systems and medical insurance processes.
  • Basic understanding of patient care, medical terminology, and HIPAA requirements.
  • Prior experience supporting behavioral health or psychiatry providers, or demonstrated experience coordinating care for patients with psychiatric or behavioral health needs, is strongly preferred.
  • Experience with psychotropic medication monitoring, GDR workflows, or long-term care regulatory processes is preferred.

Pay: $25.00 - $30.00 per hour
