Medical Nurse Practitioner (NP) / Physician Assistant (PA)

The Post-Acute Care Nurse Practitioner (NP) provides comprehensive, patient-centered medical care to individuals residing in skilled nursing facilities, long-term care, assisted living, and other post-acute care settings. This role focuses on managing chronic disease, addressing acute medical issues, supporting transitions of care, and collaborating with interdisciplinary teams to ensure high-quality, compliant care delivery.

DIRECT PATIENT CARE: Perform comprehensive-medical assessments, diagnose and manage acute and chronic conditions, develop care plans, order diagnostic tests, manage medications, and conduct regulatory-required evaluations.

CARE TRANSITIONS & COORDINATION: Manage new admissions, hospital readmissions, coordinate care with hospitals and specialists, and participate in interdisciplinary meetings.

DOCUMENTATION & COMPLIANCE: Complete accurate documentation, support QAPI initiatives, and participate in audits.

COLLABORATION & COMMUNICATION: Work with physicians, facility staff, and families to support patient-centered care.

QUALIFICATIONS: Active Nurse Practitioner license, national certification, DEA eligibility.

PHYSICAL & WORK REQUIREMENTS: Ability to stand, walk, and lift up to 25 pounds. Travel between facilities may be required.

Job Type: Full-time

Pay: $100,000.00 - $140,000.00 per year
