Medical Provider- Integrated Primary Care MD and/or DO

Description:

Medical Provider - Family Nurse Practitioner (FNP) to provide comprehensive primary care services within our integrated outpatient behavioral health model. This position is based primarily at the Mofford Integrated Clinic and serves patients enrolled across Axiom Care programs through a mix of in-person and telehealth visits.

The FNP functions as a core member of an interdisciplinary care team, collaborating closely with behavioral health clinicians, nursing staff, care coordinators, and peer support specialists to deliver coordinated, whole-person care to a complex and underserved patient population.

Duties/Responsibilities

  • Provide comprehensive primary care services, including assessment, diagnosis, treatment, and management of acute and chronic medical conditions
  • Conduct patient visits in-person and via telehealth, as clinically appropriate
  • Collaborate closely with behavioral health clinicians and other care team members to support integrated treatment planning
  • Participate in daily team huddles, care conferences, and interdisciplinary meetings
  • Prescribe medications and manage medication regimens in accordance with Arizona scope of practice laws, collaborative agreements (if applicable), and organizational protocols
  • Order, review, and interpret diagnostic tests and laboratory results
  • Maintain timely, accurate, and compliant medical documentation in the electronic health record
  • Follow all applicable federal, state, and payer requirements, including AHCCCS/Medicaid regulations
  • Support quality improvement initiatives and implementation of integrated care best practices
  • Seek consultation from supervising or collaborating physicians as clinically indicated

Requirements:


Required Skills/Abilities:

  • Ability to deliver high-quality primary care services within an integrated, team-based clinical model
  • Strong clinical assessment, diagnostic reasoning, and treatment planning skills for patients with complex medical, behavioral health, and social needs
  • Demonstrated ability to collaborate effectively with behavioral health providers, nursing staff, care coordinators, and peer support specialists
  • Proficiency in electronic health record (EHR) systems with the ability to produce timely, accurate, and compliant clinical documentation
  • Knowledge of and ability to comply with Arizona scope-of-practice regulations, professional standards, and organizational policies
  • Familiarity with AHCCCS/Medicaid requirements, quality metrics, and documentation standards preferred
  • Strong verbal and written communication skills, including the ability to engage patients from diverse backgrounds with empathy and professionalism
  • Ability to manage time effectively, prioritize competing demands, and maintain productivity in a fast-paced clinical environment
  • Sound clinical judgment with willingness to seek consultation or escalate care when clinically appropriate
  • Commitment to quality improvement, evidence-based practices, and continuous professional development
  • Ability to maintain confidentiality and comply with HIPAA and privacy regulations
  • Comfort providing care via telehealth platforms, as clinically appropriate

Education and Experience:

  • Licensed Medical Doctor (MD) or Doctor of Osteopathic (DO) in the State of Arizona
  • Active DEA registration (or eligibility to obtain)
  • Experience in primary care, family medicine, or community health preferred
  • Experience in integrated care, behavioral health settings, or serving complex populations strongly preferred

Physical Requirements:

  • Ability to sit, stand, and walk for extended periods throughout the workday
  • Ability to move throughout a clinical setting to interact with patients, staff, and equipment
  • Manual dexterity sufficient to perform clinical examinations, procedures within scope, and use medical instruments
  • Ability to lift, carry, push, or pull objects weighing up to 25 pounds, such as medical supplies or equipment
  • Visual acuity sufficient to read patient records, diagnostic results, medication labels, and computer screens
  • Ability to hear and communicate effectively with patients, staff, and interdisciplinary team members in person, by phone, and via telehealth platforms
  • Ability to use computers, keyboards, and standard office and clinical equipment for documentation and clinical care
  • Capacity to perform duties in a fast-paced clinical environment while maintaining focus, accuracy, and professionalism


Scope of Practice & Regulatory Compliance:

Clinical practice is conducted in accordance with Arizona licensure requirements, applicable state and federal laws, payer standards, and Axiom Care policies.

  • Medical Doctors (MD) and Doctors of Osteopathic Medicine (DO) practice independently within the scope authorized by the Arizona Medical Board or Arizona Board of Osteopathic Examiners and may provide clinical oversight, supervision, or consultation to other licensed providers as required by organizational structure, payer standards, or regulatory requirements.
  • All providers are expected to comply with credentialing, re-credentialing, peer review, and quality assurance processes.
